online account with DeKalb Planning & Sustainability at this link:

You will then submit your application (and all the documents listed below)
online using the URL listed above.

Be sure to upload the following:
Your Membership Agreement
A copy of your driver’s license
Evidence of your state registration (if applicable); ex your Secretary of
State LLC or Corporation registration
Your DeKalb Health Department OR Georgia Department of Agriculture
license or inspection form
The attached packet containing PREP’s CO, FOG permit and sanitation
account information
Your occupancy permit packet (PREP2 AND PREP3 MEMBERS ONLY!)
NOTE: When you type in PREP’s address, enter the first word of the street
name only. For example, shared kitchen users would enter 3300 Marjan.
PREP2 Members would enter 3795 Presidential, and PREP3 Members would
enter 3781 Presidential.)
IF YOU ARE A FOOD TRUCK OPERATOR:
Do not mention that you are a food truck when you submit your business
license application.
DeKalb County does not issue business licenses to food trucks—tell them
that you are a caterer.
